Application Forms for EC2

Please complete and return the following items:

Application for Admission:  A separate application must be completed for each child.  Please complete every item and attach a recent picture of the student.  Please remember to sign the back side of the application.

Permission for Release of Records: Complete the items in the box at the top of the page only.  Return this form with the application.  The only grade exempt from this requirement is EC2.

Teacher Recommendation: Complete the items in the box at the top of the page only.  Return this form with the application.  This form is required for all Early Childhood students who have been previously enrolled in another program as well as for all elementary and middle school students.

Application and Testing Fee: Please see the Tuition Schedule tab under “Admissions”.  Please make your check out to Hillel Day School of Boca Raton.  This fee is non-refundable.

Please note: The application WILL NOT be processed without all of these items.
